DaVita IKC's integrated care team partners with you, your caregiver, your doctors and your other healthcare providers to coordinate your care and help you focus on your health and living life to the fullest. Our services are available to eligible people, in partnership with your health plan or accountable care organization (ACO), at no cost to you.
