Q. How do I become eligible to participate in the DaVita® Integrated Kidney Care (IKC) program?

A. If you have kidney disease and participate in a health plan or ACO, you might be eligible for DaVita IKC. To find out if you are eligible for DaVita IKC, call 1-833-IKC-DAVITA (452-3284), TTY users: 711. We are available 24 hours a day, 7 days a week to assist you.

 


 

Q. Do I have to pay anything to participate in the DaVita IKC program?

A. No. The DaVita IKC program is provided by participating health plans and ACOs at no cost to people with kidney disease or members who meet the eligibility criteria for the program.

 


 

Q. Will I still be able to see the same doctors and dialysis clinic?

A. Participation in the program does not require you to change doctors or dialysis clinics, provided they are participating providers under your health plan or ACO. Please refer to your health plan or ACO for a current list of participating doctors, dialysis clinics and other healthcare providers.

 


 

Q. Will my doctors know I'm participating in DaVita IKC?

A. Yes. Your Integrated Care nurse, in partnership with your health plan or ACO, will contact your doctors to tell them you are participating in the program.

 


 

Q. When should I call my Integrated Care nurse?

A. Call your nurse when you:

  • Aren't feeling well and need help
  • Have questions about your medicines, foods, and treatment choices
  • Don’t understand your doctor’s plans for your care
  • Need help scheduling vaccinations and routine check-ups
  • Want help asking about your health plan or ACO benefits
  • Are going into the hospital
  • Want to know more about diabetes, high blood pressure, the kidneys and your medical care

Q. What do I do in an emergency?

A. In an emergency, call 911 and get help at the nearest hospital. After you get treatment, you can reach an integrated care nurse 7 days a week, 24 hours a day by calling 1-833-IKC-DAVITA (452-3284), TTY users: 711. Be sure to tell all your doctors, your dialysis clinic and DaVita IKC about your emergency care.

 


 

Q. How do I use DaVita IKC services?

A. Simply contact your integrated care nurse or care coordinator. If you’re not sure who your nurse or care coordinator is, call us at 1-833-IKC-DAVITA (452-3284), TTY users: 711. We are available 24 hours a day, 7 days a week to assist you.

 


 

Q. How does the DaVita IKC program use my personal health information?

A. DaVita IKC requires the same access to information about your health as your doctors, health plan or ACO. We strictly follow HIPAA requirements for the safety and security of your personal health information. We will not use your information for any sales or third-party marketing activities.

 


 

Q. Will my eligibility to receive a transplant be affected?

A. No. We encourage you to complete evaluation and testing to remain active on the transplant list.

 


 

Q. Does DaVita IKC promote specific products?

A. No. DaVita IKC has no financial arrangements with other organizations to advertise, market or promote products or services. DaVita IKC does not advertise, market or promote specific products or services to patients or providers.
