DaVita® Integrated Kidney Care (IKC) works with you to align and coordinate your patient's care beyond the walls of the office and the dialysis clinic. Our integrated care team of specially trained renal nurses (RNs), nurse practitioners (NPs) and care coordinators work with physicians, their practice teams and dialysis clinics. Together, we integrate care before and during the transition to dialysis or transplant for patients with late stage CKD, and we manage care inside and outside the clinic for patients with ESRD who depend on life sustaining dialysis.

DaVita IKC has served patients for two decades. Today, we positively impact the lives of more than 25,000 patients each month.

Because this program is available to eligible patients in partnership with their health plan or accountable care organization (ACO), there’s no cost to participate. DaVita IKC is provider neutral and works with patients regardless of their dialysis provider.
